CPCW serves as a referral network connecting clients with independent mental health and wellness providers. Please read these terms carefully.


1. Role of CPCW
CPCW acts solely as a referral platform and does not provide or manage mental health or wellness services. All services are delivered by independent providers who are not employees, agents, or contractors of CPCW. CPCW is not responsible for the quality, outcomes, or conduct of any providers or services. Clear Path Community Wellness (CPCW) does not verify the credentials, licenses, or insurance of independent providers. Clients are encouraged to independently assess the suitability of any provider referred through the CPCW platform. CPCW will not be held liable for any issues arising from the services provided by independent practitioners.



2. Client Responsibilities
Clients are responsible for selecting their providers from the CPCW network. CPCW makes no guarantees about the suitability or availability of providers. Clients are encouraged to conduct their own assessments of providers based on their needs.


3. Provider Responsibilities
Providers are responsible for carrying their own professional liability insurance and ensuring compliance with all relevant laws, including privacy and confidentiality regulations (such as HIPAA). Providers operate independently and are not directed or supervised by CPCW.


4. Events and Community Activities
CPCW may offer community events, workshops, and networking opportunities. Participation in these events is voluntary, and CPCW is not liable for any advice, recommendations, or outcomes resulting from participation.


5. Membership Fees
Providers who join the CPCW network are required to pay a monthly membership fee. This fee begins only after the provider receives their first referral through the platform. CPCW reserves the right to terminate membership for any breach of these terms or failure to maintain professional standards.


6. Income-Based Scholarships
Scholarships are available to eligible clients based on financial need and availability of funds. CPCW reserves the right to modify or discontinue scholarship programs at any time.


7. Limitation of Liability
CPCW is not liable for any indirect, incidental, or consequential damages arising from the use of its platform, events, or services. CPCW’s total liability shall be limited to the amount of membership fees paid in the last 12 months.


8. Governing Law and Dispute Resolution
These terms are governed by the laws of the State of Colorado. Any disputes shall be resolved through mediation or binding arbitration in Jefferson County, Colorado.


9. Amendments
CPCW reserves the right to modify these terms at any time. Users will be notified of any changes through the platform.
